Instructions and Notes: Fill
out all four parts (I, II and III and IV).
If you are returning within TSPA and all the
informaiton is the same or just making up a lesson and you have
filled out a form already, only fill out part I and IV.
If you are registering siblings, fill out all
four parts for the 1st child and only part I, II and IV for the
other children. For example, once you push the submit button on
this form, click register another child in the top 10 things to
do from here on the next page.
Please note that by registering your child for
the Jr. clinics does not guarantee that space is available. When
entering 5 for number of weeks, this means you will take the same
time and day every week for 5 weeks (i.e. 5 weeks Saturday 12:00-13:00
means you are registering for the next 5 Saturdays from 12:00-13:00).
Required Fields: Please fill
out all the required fields marked with * such as child's first
name, last name, membership information and how you will pay for
the clinic, your name, email and you must check off to agree to
the TSPA terms and conditions.
Payment Information: Payment
must be received at least 14 days in advance of the clinic. In
the case that you are registering within 14 days, please make
arrangements to pay as soon as possible or at the beginning of
the lesson with the pro to finalize registration.
